My Hijack This Log: Need help with TVMedia

I ran Hijack This, and as posted on some other messages, checked the 3 entries, one for R3 tvmedia BHO, and two for R4 tvm.exe, but even after I click Fix, they still show up again. Please review my hijack this log and let me know how to get rid of these. thanks in advance:

Then reboot into safe mode and delete:
