    Well, to make an educated guess, it means that access to a particular file by the virus scanner won't count as the last time that the file was accessed.

    from the help file:

    Preserve last access timestamp:
    Check this option to keep original time of access to files instead of updating it (for use with data backup systems).

    its very simple, open the on demand scanner preferences, select treatsense setup, other, click F1 ;)
