It is literally the only icon I'm having problems with. The icon shows up in Control Panel and other Windows 10 menus, but not on the desktop. It disappears the next time I reboot. We have very occasionally had someone report this issue and, as you mentioned, the issue has often been resolved by clearing and resetting the Windows / shell icon cache. Perhaps some real-time anti-virus / security program is locking the file to scan it (and takes too long), and that leads to a timeout trying to get the icon for display. If you have such software installed, you could try adding spywareblaster.exe to any exclusion list / "whitelist" / allow list, and see if that prevents the issue from reoccurring after a reboot. Fortunately / unfortunately, since this remains a pretty rare phenomenon, a little experimenting may be required to (hopefully) figure out the cause on your system.
