A number of AV products, as well as their IS cousins, are available in 3 user editions.

    This permits 3 installs.

Must each install be in a separate OS.?

May the installs be back to back in th same OS, in effect, giving 3 years on 1 OS?

    NOD32 does not permit this.
What about Kaspersky, Norton, McAfee and Trend Micro?
    Any others?

No , the OS doesn't depend . You can install them all on XP machines as well as on other OS , however , up to 3 installed at the same time . Keep only 1 AV installed on one computer

I don't understand this part very well . Do you mean you wish to transfer 3 users licenses for 1 year into 1 user license valid for 3 years . This is sales issue (not technical) however I believe that generally this is not possible . If , however , you have stable/valid reasons to do that , some Sales Manager may do it for you , it just depends on the situation

Depends on how smart/dumb is the software.
Each install could increment a counter back at the ranch.
Only if you exceed the 3 installs would you have to call, e.g., if there is a drive problem that requires a reformat and re-install.
