Just another email hoax/social engineering attack

Received this in the mail today, in my inbox, however the images were disabled which they do for all messages.
Have to say, they're just getting more and more creative with each such identity theft hoax, had to post a complete screenshot to truly appreciate their art; it even includes a captcha field this time

My apologies if this has been posted already however i did search and didn't find it here.
