Is there anything I should do before creating an image?
Would it be better to say block all my startup items with msconfig before creating an image or manually shutting down background programs like my AV and so on?

Nope. Only if you run stuff like MS SQL Server, Oracle RDBMS, MS Exchange, or any other kind of transactional system where it is important to have the data in a consistent state for a restore to be useful.

If you are semi-paranoid about the validity of the image, reboot into Windows Safe Mode and make your image. In Safe Mode none of the stuff you mention should be active anyway.

If you are really paranoid, boot the True Image Rescue CD and make the image.
