How wise (or not) it is to exclude the c:\sandbox folder from antivirus real time?

I don't think there's any necessity to exclude the sandbox folder or Sandboxie's program folder. I have used it with MSE without any exclusions and there was no noticeable problem. Sandboxie's compatibility alert should be sufficient enough if you worry about conflicts.

Is it wise to exclude the Sandbox folder? In my opinion, Sandboxie works better on its own. I like to avoid potential conflicts that may arise between SBIE and the AV. So, for me, yes, excluding the Sandbox folder is the wise thing to do but if I was using a real time AV, like you are, then I would not exclude the sandbox folder and would let the AV do its thing.
