Activation requires sms in most cases, and the mentioned sites with fake numbers allow you to receive a message thus to see the content . As I believe you cannot be anonimous online you are at least not revealing the private number to sb. As for email a good one is with antispam, 2 factor autentication or usb key, with option to encrypt messages and some anti malware measures . I'd say 2 factor aut. is more important than not giving out a number. 2 factor with fake number is risky as you read the message from a sites rather than isolated device
Well, I can assure you that I have multiple Tutanota and Protonmail accounts and I use them since years with VPN and always with the same IP address for each session. Sometimes when making a new account with Tuta through VPN, it flags it for a 48 holding time but then it gets activated.
