My take on this is that I have never seen anything like that. At least not that all encompassing.

The registry is a living, breathing thing. There are constant changes going on all the time. The vast, vast majority are benign, harmless changes. Monitoring it all and alerting to all changes would need a tremendous amount of horsepower and IMO, be pretty much a waste of time.

Something like Registry Watcher and the like focus on harmful (or maybe better said as potentially harmful) changes. Much easier to manage and much simpler.

    My 10 cents... :)

Actually each HIPS can do this better or worse but if you want to know about changes in registry (not in real-time) and than decide you can use Tiny Watcher

Tiny Watcher can do this but after rebooting the system...it shows window with the list of latest changes in system so that you can allow or deny some listed specific entries.
    http://kubicle.dcmembers.com/watcher/misc.html#The snapshots in details
If you can only allow of deny some apps maybe only process monitors like Process Explorer, Process Hacker, System Explorer, Anvir Task Manager could be enough? But...if you want manage of behaviour of specific aplications you should rather use HIPS or behavioral blocker
I don't remeber open source HIPS but free you can find on this site
