Is a trojan (malware) setting in C:\WINDOWS\SYSTEM32. Have you been able to quarantine the trojan ? .....It could be a variant of this type : http://www.eset.com/us/threat-center/encyclopedia/threats/win32trojandownloadervidloq

Make sure when you are doing a manual scan from the right-click menu that you go to Advanced Options, Clean files... instead of just scanning. Since the real-time scanner isn't yelling at you constantly about it, it probably isn't an active virus infection.
