Is this simply based of user ratings ?

not all the cloud-based AVs are based on users votes

for example kaspersky cloud system which helps the behavior blocker
but KSN is based on users ratings
or Norton reputation scan via user ratings
or Comodo cloud scanner uses the online database

and again i say not all the cloud based AVs use the users ratings.

simply "cloud" means to use online scaning

You wouldn't notice anything during daily use, the point is that everything is handled in the cloud (Collective Intelligence, Panda speak) just like with Webroot. I use both and although a longtime PrevX/Webroot user, i actually recommend PCAV Pro to my non tech friends and they like the simplicity of not having to tweak anything they do not understand anyway. Webroot i recommend for the slightly more advanced user who likes options.

Yes, it does. A small definition file is downloaded whilst online, which then helps to protect your computer when offline. If this option is disabled in Settings/Realtime Shield, you'll not get this file delivered. It is recommended to leave this enabled.
