I use the download app in Opera (transfers) and high speed cable does it for me.

Simple Mini download manager - part of IE7Pro, only because IE does not support resume and multiple downloads can take dozens windows.

Orbit Downloader over here, it was the first DM that didn´t suck IMO. I now use it once in a while for big downloads, and if I need to dowload several files at the same time. But the best part about it is the file sniffer function, I use it to mostly to download Flash videos, freaking awesome!

None for me. Back in the dial up days...they were handy..with occasional disconnects 'n such. But with full time broadband now...I haven't found the need for them.

Many years ago I used Download Accelerator (or something like that)... it was plagued wih adware.

With dialup service or any internet service with an unreliable connection, a download manager is a necessity. I used the free version of Star Downloader very heavily when I had dialup. It worked very well and is adware/spyware free. I downloaded some very large files with it over several weeks by running it at night when the phone wouldn't be in use, including 600+ MB ISO files for Linux live CDs. I don't download near as much as I used to so I haven't been using it.
If the connection is fast and stable, then a download manager isn't necessary, but they can still be useful. Star Downloader for instance integrates with most AVs and can automatically scan downloads. If you need to reboot, you don't lose what you've already downloaded. The ability to schedule downloads can be useful if your connection isn't real fast.
