NOD32 v4 is finding and deleting this worm but it is continually reappearing - any ideas anyone?

are you using a firewall? it must be on your ISP network and gains access to your pc. i had alot of problems removing conficker worm...i didn`t use any firewall, my av detected it, cleaned it but after each restart it reapeared...

I'd suggest posting here the relevant records from the Threat log so that we know what is detected and where it's detected.
