How do you manage client replacements?

How do you guys manage your client list? Our clients are continually changing due to upgrades or replacement of old machines. There is an entirely different group making these changes and none of the changes are reported to me.

How do you manage your old clients? It is cumbersome to filter out the legitamite problem clients and clients that are just no longer there.
