If I make a payment online with my credit card, usually you have to put in
an address that goes with it, even if it is just for an online service.

Does this address get seen by the service provider?

On the page where you enter your address, is there a little padlock on the browser? If yes, then that page is encrypted and can't be seen by the ISP. They will be able to view any pages that are not encrypted (no padlock). They will also know what website you visited (encrypted or not).

Sorry, I didn't make myself clear enough. I don't mean internet service provide. I mean if, for example, you wanted to open an account for Google Adwords, does Google get to see your address details that go with the card?

Yes, Google will see your billing address because they will need your billing address to charge your credit card.
