Does the program have to be open to work? Will it start with window?

Do you mean Javacool's "Spyware Blaster" program? If so, no it does not. It works by setting values in the registry that prevent specific ActiveX controls from running (or even downloading, if they are not already on your system). Therefore, it does not need to run continuously.
Can you confirm this is the software you are talking about?

Yes, that's the one. Here is a thread that talks a little more about how to use Spyware Blaster:

Basically, you simply need to run it occasionally, (to pull down any spyware definition updates that Javacool puts out), have it set the registry entries (kill bits), and you are done. How often you run it to look for updates is up to you, but, I'd do it weekly, at the least.
